We collect information that personally identifies you only when you provide it to us by filling in an online form. This information may include your name, email address, address, and telephone number. Mobile information collected, including information provided for receiving SMS messages, whether sent via standard long-code numbers or via our toll-free SMS number (1-800-228-6483 / 1-800-22UNITE), will not be shared with third parties or affiliates for marketing or promotional purposes. All identifiable personal information that you provide will be kept confidential and will only be used by authorized individuals to fulfill the purpose for which it was originally collected or for a use consistent with that purpose. All the above categories exclude text messaging originator opt-in data and consent; this information will not be shared with third parties.
