"National winter and summer schools, as well as deepening classes at the local level, are an integral aspect of community life in which the friends come together to deepen their knowledge of the Faith. Institutes were created as an instrument to develop the human resources necessary for the expansion and consolidation of the Faith. These two elements are complementary, and benefit from each other as they both flourish."

All contributions will go to the National Baha’i Fund and Green Acre Scholarship Fund. Please advise in advance by phone or email if you are bringing items of value for the silent auction. There is no charge for the dinner, but donations are welcome.

(Friends of all faiths are welcome. Please remember that only Baha’is may donate to the general Baha’i funds.)

How to Complete the Online Medical Release Form for your Child

Are you planning to register your child for Badasht Prep, Camp Green Acre, Badasht Academy or other youth institutes?

Your registration will not be complete until the online medical release form is completed!
The medical release form must be completed even if you will be at Green Acre during your child's program, since your child will stay with a counselor and group of peers.
If you can't complete the form online, call our Registrar and he will go online and do it for you, while you are on the phone.

The questions you will have to answer are below. You may write "none" if that is the case.

Medical Release Forms Questions:

1. Any known allergies or health problems?

2. Any known reactions to medications?

3. Is your child currently taking any medications?

4. Date of last tetanus inoculation.

5. Name of your child's physician and phone number

6. Medical Insurance Provider and Policy Number

7. Primary Insured's Name and Date of Birth

8. Child's Date of Birth

9. To indicate your consent to the statement above, please enter yes, and type your name which shall constitute an electronic signature. (This is a yes/no question that you will do online.)
